Bleeding Hearts Interest Form

    We will be determining costumes and room assignments for this FIRST Valentine's Day Haunted House. Most costumes will be provided.
    If you have not participated in a previous year we will have an orientation prior to the first night that you will be required to attend. 

    ​I have read everything and understand. If under the age of 18 I have parental or guardian approval to participate. Bleeding Hearts will take place February 13 and 14, and understand that I should plan on arriving 45-60 minutes to the start each night. I have correctly selected the nights I am able to volunteer.
